Patient Portal Sign Up

If you are an enroled patient and would like to sign up to the patient portal you may use this form. Patient portal allows you to book appointments, request repeat prescriptions and see your health summary. Sign up does not happen automatically as certain manual checks need to be made.

Accounts are only available for those 16 and over.

To prevent identity fraud we require you to upload a copy of your up to date photo ID e.g. your passport or drivers license. If we have any issues we may contact you or ask you to come in to complete the sign up process in person.

Your email address will be your username, therefore each adult requires their own email address to sign up. Personal rather than work email address is preferred, as you are accessing personal health information.
